Module Aims:
To develop an understanding of the impact that long term conditions have on patients’ physical and psychosocial well being.
To develop awareness of the health care resources available to provide appropriate and informed nursing care and support for those with long-term conditions.
Learning Outcomes:
Understanding of the role of the nurse in meeting care needs of patients with long- term health conditions, applying knowledge relating to assessment data, in order to plan safe, appropriate and individualized care.
To be able to formulate appropriate plans of care for patients with long-term conditions, using integrated care planning and/or care pathways, recognising the importance of the roles of others in multi-professional care.
To be able to analyse the use of national service frameworks, government policies, self-management and current evidence which informs the management of patients with long- term conditions.
To be able to apply a legal and ethical framework and a non-discriminatory approach to analysing situations from practice.
