Normalization of Data

The transition into digitized data storage and access systems in health care requires a general adjustment in data capturing techniques. In the days of paper records, redundancy was a standard—information was written and rewritten with each patient visit. Digitized data storage and the normalization of data can help reduce this redundancy. Ideally, a patient can enter a facility and information previously submitted is readily available. The provider can simply apply updates and make adjustments to the information as necessary. This saves the health care provider time and the organization’s information system space. Additionally, this decrease of data input can lead to improved quality and patient safety as fewer diagnostic errors associated with navigating a patient’s information are less likely to occur.Consider the following scenario:You have recently been hired by a small community hospital as a nurse informaticist. One of your first responsibilities is to help to convert records to an electronic format. You decide to address the process of ordering medicine for patients and you need to develop a database to address the issue. Currently all of the patient information is contained in an Excel spreadsheet, which contains the following categories: PATIENT ORDER, PATIENT medical record number (MRN), PATIENT Name, Order Number, MEDICATION Name, MEDICATION , Quantity, PATIENT Address, and Date Ordered. In designing your database, you need to normalize the data in order to remove redundancies and duplications. What approach will you take to normalize the data?Note: A PATIENT can have multiple orders but an order can be for only one MEDICATION. Patient medical record number (MRN) and Order Number are the primary keys.To prepare:Review the information in your course text, Coronel, C. & Morris, S. (2015). Database systems: Design, implementation, and management (11th ed.). Stamford, CT: Cengage Learning, on how to normalize data.Refer to the scenario above and determine how you would normalize the information (there is not just one correct way of doing this). What form of normalization would use (1NF, 2NF, 3NF)?Illustrate the normalized form for PATIENT ORDER.Reflect on the issues you encountered as you attempted to normalize the list.Consider the consequences of not normalizing data.Post on or before Day 3 a brief description of how you would normalize the data from the scenario and your rationale. Describe the challenges that you encountered in determining how to normalize the data. Explain the possible consequences of failing to normalize data.

Business Rules and Data Models

Students, please view the \”Submit a Clickable Rubric Assignment\” in the Student Center. Instructors, training on how to grade is within the Instructor Center. Assignment 1: Business Rules and Data Models Due Week 2 and worth 100 points Suppose a local college has tasked you to develop a database that will keep track of students and the courses that they have taken. In addition to tracking the students and courses, the client wants the database to keep track of the instructors teaching each of the courses. Use the internet to research the types of information that a college would want to track in a database. Focus your attention on information that could represent entities within a database as well as attributes of such entities. Write a two to three (2-3) page paper in which you: 1.Describe the purpose of the database.. 2.Based on your research and / or common knowledge of a college and the information tracked by a college, describe the entities of the database and the attribut

Mental Health Case Study: Implementation and Evaluation of Nursing Care

ESSAY BRIEFINGThe focus of the case study must be on the implementation and evaluation of the nursing care and include physiological, pharmacological, and psycho-sociological aspects of your care of a patient/client during one shift of duty, or single significant episode of care such as home visit, outpatient visit or similar therapeutic intervention. You are required to:• Demonstrate logical, coherent development in your nursing case study. You are writing the nursing care of a patient, with a condition, you are NOT writing the medical management of a condition: remember it is NURSING care.• Include an introduction and a conclusion. Avoid a long description of what happened, seek to give why events occurred and what were the nurses trying to do.• You may present the case as you wish, for example you may give priorities for those elements that were a challenge to you or the nurses. Use professional language, and proof read your work before submission.• Bullet points should be avoided, however you may use subheadings.• Avoid use of direct quotes unless they are absolutely essential and add meaning to your work.• Provide a concise introduction to your patient and their current health status. It is in this introduction to your patient or appendix where you can refer to the assessment and planning which preceded the implementation, but keep this brief.• Only analyse and explore needs which are relevant for the timescale covered. You may include a more detailed synopsis in appendix if relevant and you must refer to the appendix. You will not gain any marks for including an appendix.• You must demonstrate evidence of relevant reading from academic sources and use this to support your work. DO NOT just reiterate what you read, but be critical of claims made and how this influences your thinking. For example if you are referring to a patient’s pyrexia, you must consider what the normal values are and how your management is guided by evidence.• Consider psycho-social issues that you needed to take into account when implementing care. If you are studying to be a mental health nurse, you may wish to consider a patient/client’s behaviour and the care that you would implement in these circumstances. Consider cultural issues which may impact on your care and any pharmacological interventions required.Case Study: 84 year old male patient deatained under Section 3 of the Mental Health act (1983) with vascular dementia, poor mobility, memory loss, aggressive behaviour, impaired vision and cellulitis of the leg.Comments from previous marker:What you have done well: You have clearly put much effort into the assignment and attempt to adopt a scholarly approach. What you have not done well: You have changed the nature and focus of your assignment. However, many of the criticisms of the first submission remains. There is still very little focus or analysis of the implementation and evaluation stages of nursing care – as this is the focus of the assignment this presents a challenge to satisfy the demands of the assignment. You still primarily focus on the patient rather that nursing care. There are at times unreferenced statements and unsubstantiated assertions. Also there are times when your proofreading is not all that it should be – for example, I did not the that a \’men mental health act\’ existed. The work terminates abruptly and appears unfinished. How to improve in future: Ensure that you first submission addresses the task set. Focus is all at this and subsequent levels.

Workflow Redesign

After conducting a thorough gap analysis, the next step in the systems development life cycle (SDLC) is to target potential solutions to the gaps. There may be many potential solutions that can help to address workflow issues and inefficiencies, or there may be one seemingly obvious solution that could address almost all of the gaps in the current-state workflow. The challenge lies in selecting the most appropriate course of action from potential solutions that also works within organizational constraints. In this Discussion, you revisit the scenarios from the Week 3 Discussion. You determine the possible avenues for workflow redesign and consider the constraints and factors that might impact your decision.Scenario 3:Robert works in the medical records office at Garden View Hospital. Because the hospital does not have a policy of providing clinical summaries to patients after each visit, Robert frequently receives calls from patients requesting copies of their health information (such as test results, vital statistics, diagnostic information, and medication lists and dosages). For every request, Robert must retrieve the patient information from the hospital’s electronic health record (EHR), print out hard copies of the records, and then either mail the records or wait for the patient to retrieve them in person. Additionally, Robert’s hospital often receives lab test results for patients from certified third-party providers. These results are not automatically transferred from the provider’s EHR to the hospital’s EHR, so Robert often must contact the providers before responding to a patient’s request. Robert also finds that he and his colleagues in the office spend a considerable amount of time printing, mailing, and faxing immunization records to patients’ workplaces, schools, and volunteer organizations, which can be disruptive to the office’s other responsibilities. To prepareReview this week’s Learning Resources on workflow redesign. Then select one of the scenarios listed above to consider for this Discussion.Note: You may choose to examine the same scenario you were assigned for the Week 3 Discussion, or you may select a different scenario.Identify one specific workflow issue (gap) in the scenario you selected. Formulate at least two distinct solutions for this workflow issue.Of the two solutions you developed, which would you recommend for redesigning the current state workflow in the scenario you selected? Why?Reflect on the solution you would recommend. What two organizational constraints (budget, time, resources, personnel, IT expertise, resistance to change, lack of leadership, stakeholder buy-in, etc.) would most significantly impact the implementation of the solution? How would they impact the solution?By Day 3Post the scenario you selected and a description of the two solutions you developed for this workflow issue. Identify the solution you would recommend, and explain how this solution would address the workflow issue. Identify two organizational constraints that might impact the workflow redesign and explain their impact.
