Purpose:
The purpose of this assignment is to allow the student to reflect on their role in population health and demonstrate competency in population health about the course objectives.
Review the Module 5 learning activities for this module. Create a 2-page paper that addresses the following:
Reflect on your learning this semester and how the social determinants of health affect your current practice.
Describe how your perception of population health and community health nursing has changed.
Propose your potential personal and professional impact on population health in social determinants of health, chronic care management, safety, and community health.
Also only if you needed it must cite and reference at least one professional or peer-reviewed source (a journal article published within the last 5 years) in APA 7th edition.
